At Evergreen Psychiatry, we assist to complete a variety of educational and employment - related paperwork.
We assist with letters to
- Initiate Individualised Educational Plans (IEP)
- 504 plans for school.
- letter for confirmation of diagnosis.
- Psychological evaluations.
- Letters of recommendation and FMLA forms.
- Other documents you may provide.
