If the patient is underage, kindly fill out our CONSENT TO TREAT A MINOR form here. This document must be filled out, signed, and witnessed before the patient's initial appointment to prevent any delays in treatment or rescheduling.

IF THERE ARE ANY COURT, CUSTODY, GUARDIANSHIP, OR HEALTHCARE PROXY FORMS, PLEASE KINDLY PRESENT THEM AT THE INITIAL APPOINTMENT; OTHERWISE, THE PATIENT MAY NOT BE SEEN.

Insurance Options

We accept most Commercial Insurance; however, please call your insurance company to confirm that we are in-network before scheduling an appointment. We accept most North Carolina-managed Medicaid Plans.

If we cannot confirm your coverage prior to your appointment, you will have the choice to reschedule or pay the self-pay rate until your insurance is verified.

Evergreen Psychiatry Group offers outpatient clinic services only with limited office hours.
Patients are seen on an appointment basis only. In case of an emergency, please browse our emergency resources to find a service that suits your needs.

Guilford County Human Resources

For 24/7 walk-up access to mental health services for Guilford County children (4+), adolescents, and adults, please visit the new Guilford County Behavioural Health Center for immediate assistance with mental health and substance abuse issues.

Wake County Human Services

Wake County Human Services consolidates programs including social services, public health, mental health, job training, child support, housing, and transportation. They can provide information and assistance regarding county mental health programs.
